Passwd/LDAP problem

I have openldap 2.0.27 on RH 8

I have been running this for about 4 months and have just had the first
account be prompted for a password change.

However, the problem is that this account is being prompted to change their
password at every login since.

If you've seen this before can you point me in the right direction to fix
it?
