Here is quick survey about HIPAA.

Anonymous? Only if the site visitor anonymizes prior to visiting. Most web
servers capture client IP info upon visit.

Regardless, you peaked my interest and I checked it out... pretty neat...
how did you setup the results part? I would like to do something like this
in the future and would love to learn how to make my results look nice like
yours.

Your first question should use radio buttons, not checkboxes, as with the
second question.
