What ports do I need to forward and How do I configure

Port 25 (SMTP) and port 110 (POP) open to the Exchange server, port 80
(HTTP) open to the web server, and port 53, both UDP and TCP, open to
both servers and your DNS server.

As for configuration of Exchange, it depends a little on your version,
for 5.5 there's a how-to in the FAQ:
