IIS Index Server - ISS search page how to

Where can I find a simple IIS search page for an index catalog? (it should
be able to search for documents .doc/.txt/.zip/.xls)

I thought it was included with ISS 5 but I can't find it...

Thanks George, but would it be possible to post this code such that the
