Microsoft Content Management Server - User access not working

Running MCMS 2002 SP1a. I have a user account defined as
guest in the SCA. I have the same user account in a user
rights group with Subscriber role access to all channels.

IIS has anonymous and windows authentication mode enabled.

If I try to open the website in IE as a user who is not a
member of any rights group I am prompted for
authentication.

When I turned off anonymous access in IIS it fixed the
problem but now it has come back for no apparent reason.
Now the site is not accessible to users who are not
specified in a rights group, regardless if IIS anon is
turned on or off.

What do I need to do to get this to work?
