Are you using any other custom pipeline component written
in VB?

If yes is it poolable ?

I have seen people have similar kind of problems if they
use a custom pipeline component made in VB 6.0 and made it
poolable. By making the component non-poolable, this
problem has been resolved for some of them.

I havn't experienced this problem but may be it give you
some direction.
