Author prompted for password when accessing shtml.dll

Whenever a FP form page attempts to access /_vti_bin/shtml.dll the web
server prompts for a password. If I type in an administrator
username/password, it works fine.

I've tried giving the IUSR account full (RWXD) permissions to the 50\isapi
folder, no difference. Anything else I should check?

Hi Tex,

Create a new site and create a new form in that site. Check whether that
is working. If that is working,

If your site is extended with FPSE 2002, then check whether any roles are
present in that site. If not, add the browser and administrator role and
add one user for each role.

Then run the check server health (Please note that the check server health
will remove any permissions which is not set using FPSE)
