Trouble with server extensions

| I have a Windows SBS 2003 standard server with a website on it. I have a
form that asks for user information. When I fill out the form and submit it,
I get the following error.
Cannot run the FrontPage Server Extensions on this page:
http://www.floridatruck.net/serv05.htm
Any help appreciated. This is a new server installation, so it may be setup
related.

Open the IIS Manager console and expand the IIS server so that you see
the three folders under it. Double click the Web Service Extensions
folder and check to see if FP Server Extensions are in the list and
allowed. If not, then you will need to uninstall the IIS component of
SBS2003 and re-install it, making sure that FPSE is checked. Expand
everything until you find it and check it.

That seems to be OK. What is missing is the Indexing Service. But I
don't know how that is really set up in SBS2003 standard.
Back to the current problem. You did not say if this site was on its
own Virtual Web Server. If it is, you need to make sure that the site
is running, and that it has been configured to use the FP server
extensions. If it is on the default web site, then all bets are off.
WSS takes over the default web site and creates the companyweb site as
a host header site and is extended with SharePoint Services. The rest
of what was on the default web site appears to be on another web site,
but it is really the same site but extended with FPSE.
