Why is my FP form's email results only giving fields, not data?

| Using FrontPage 2003, I've created a simple registration form on my webpage,
similar to one several months ago that worked fine. (I do have the FP server
extensions installed on my webhost.)
This one, however, sends the email, but only showing the fields, not the
data I've entered on my test run. I've deleted the form twice & rebuilt it,
paying close attention to the Form Properties page.
Why would it send the email results message without the data?

- try plain text
