Author Getting form to work with extensions already installed.

When I click Submit Feedback on my form I get a page that says Front Page
Run-Time Components Page:
"You have submitted a form or followed a link to a page that requires a web
server and the FrontPage Server Extensions to function properly.

This form or other FrontPage component will work correctly if you publish
this web to a web server that has the FrontPage Server Extensions installed
Click the Back arrow to return to Previous Page"
Both my host and I have uninstalled and reinstalled the server extentions
then I re-published my site to no avail. Also, on my form page it has a
sentence in brackets that says [FrontPage Save Results Component] - I don't
know where that came from and I don't know how to get rid of it.
Any help would be very much appreciated.

1. Did you have a web / site open in FP via File Menu | Open Web / Site first?

2. If yes to the above, then did you publish the form to the live/remote server at
http://www.bestcaliforniarealty.com via FP's publishing command?

Ok, you opened the live/remote site (http://www.bestcaliforniarealty.com) and then you published it
where?

You should have open you local copy at C:\... or http://localhost in FP and then publish that to
http://www.bestcaliforniarealty.com

Can you please elaborate, I'm not sure what you suggest I do. Please explain
the difference in your last instructions. If your saying that I open FP on C
drive then hit Publish web from file menu, I've done that many times.

Based on what you wrote, you open your live/remote site at http://www.bestcaliforniarealty.com and
then you published it somewhere, however that would not have cause you to loose your theme,
navigation and shared borders, so I am try to figure out what you did to cause this problem, as my
instructions would not have cause this.

I re-published from FP on C drive and now the web site is back but I'm still
at square one with the form not working. I published to ftp://68.178.211.10 -
this came from my host (GoDaddy)

Okay, I'm publishing to http://www.bestcaliforniarealty.com and FP is stating
Publishing Web Structure: FP has detected that changes you have made to the
navigation structure conflict with the changes another author has made to the
web you are publishing to. What do you want to do?
Then it pre-selected Let FP merge the changes
If you let FP merge the changes, it will publish the navigation structure
but will only replace another authors change if yours is more recent"
Should I opt for that one? the other two choices are:
Do not replace the navigation structure of the destination web and
Replace the navigational structure of the destination web
