error in HWS - Failed to retrieve activity flow information from

When I start activity flow it throws following error
Failed to retrieve activity flow information from tracking.

Any thoughts?

Are you passing a non-empty guid for the parentActionInstanceId? This
parameter, and the parentTaskId parameter should be the empty guid for when
you are initiating the first action of an activity flow.
