Is there a way in BTS 04 to auto add the UTF-8 or UTF-16 encoding
reference at the top of an mapped outbound doc?

This is done by the XmlAssembler component (Add XML declaration)
Either from a custom pipeline or using the
Microsoft.BizTalk.DefaultPipelines.XMLTransmit pipeline
