Author Transformation fails Send Port: The root element is missing

I am having a problem with a simple test transformation and send that
doesn't seem work.

In HAT, I see an error and it says, There was a failure executing the
send pipeline: "Microsoft.BizTalk.DefaultPipelines.XMLTransmit"
Source: "XML assembler" Send Port: "C:\MyPorts\ExecOut\Exec.xml"
Reason: The root element is missing.
The root element is missing.

If I change the port to passthru, I see a the output file is written
but is blank - I don't see how that could be.. there is incoming data
and it is mapped to an output schema

How do I debug this further and solve for the problem?
