Squid - Static Proxy Password

I would like to configure squid so that is asks a user for a pool of
static user/passwords.

I do not want to check against the passwd file, I do not want to check
against an NT controller, I just want to say that any user who wants to
get out onto the web must enter a specific user/password.

Can anybody show me a sample squid.conf that does this?
