What does it mean when all three lights in front of an SDLT320 tape drive
are blinking, with the tape inserted?

It means a fatal drive error or POST failure. A typical cause would be
a dropped leader. You can try emergency eject: Press and hold the eject
button for (IIRC) more than 10 seconds.
The drive will have to be replaced/serviced.
