Is it possible that the system is trying to send MoH to the user(in
queue), but there is none specified/unavailable codec or the MoH is
not making it to the gateway/phone?

Not sure if that is the issue or not..

Also, does it matter if you dial from an IP phone vs. through a
gateway?
