This sure looks like I've hit some sort of IOS bug.

Run the router with 12.4.10a - doesn't work. Never gets out of
TEI_ASSIGNED / AWAITING state.
Run the router with 12.4.4XC5 - works. Can get inbound calls.

Same config, just changing the boot system flash command and reloading.
