I ran into this issue once before. The problem was that the credentials
being provided to the IPCC Express server to log onto the Callmanager server
were incorrect.

Verify that the user id and password you're using is correct, and you might
also take a look at the application event logs on both servers.

I ran into the same problem, make sure the CTI RP is in the same
Device Pool that your CTI ports are in.
