Anyone ever see unity accounts in an unified setup just go away on their own?

The exchange mailbox and AD account are there still but the Unity account vanishes out of the blue. We're seeing this on 2 seperate 4.2(1) servers and have been working with TAC. No errors in the event logs indicating a problem, etc.

Regards, Erick

Only time I've seen it was when the exchange mailbox or AD account were
deleted.
