Keep getting DuplicateKeyException for UserRegistrationCmdImpl

| Hello All,
Please help, I am keep getting this javax.ejb.DuplicateKeyException for UserRegistrationCmdImpl. I have no clue of why this is keep happening. Can someone please shine some light on this? I am working on WCS 5.1. Thank you very much.

| I forget to mention that the error occurs when I call UserRegistrationAdd url command to add a new user to the system.
Example:
