after few packs of cigarettes and coffee
finally I found solution:

the problem was that all registry and files during web extension where
created and modified by network service:

EG:

*  w3wp.exe:7072 CREATE   XXXX:\Documents and Settings\All Users\Application
Data\Microsoft\Web Server Extensions\50\owsuser.lck ACCESS DENIED NT
AUTHORITY\NETWORK SERVICE

*------------------
Cannot open file "roles.ini" for writing.

and so on....

the problem was in Microsoft IIS Administration site.. someone deleted
Application  =)
if you have similar problem just create app pool in MSsharedAPPpool and
that's it ;)


ok some advices:

1. try searching Google and news groups for information
2. if 1st advice wont help (like this time) use some tools for registry and
file low level monitoring
I'm using www.sysinternals.com freeware tools
3. technet and msdn is your friend ;)
