POST /_vti_bin/shtml.dll/_vti_rpc HTTP/1.0

This is the what is failing.  I see it in my sniffer.  The IIS server is
responding with:
HTTP/1.1 500 ( The request is not supported.  ) 536 0.080102 2.893699

You can connect with Visual Studio 6 outside the ISA but through it you get
this.  Connecting through ISA using IE works fine.
Nothing logged in IIS, on the DC or on ISA for failing although I do see in
ISA the connection attempt fails but, the first GET is successful and the
first POST is successful.  The next one fails and then the IIS server
disconnects.

The file: shtml.dll is in the virtual directory /_vti_bin, physical address
C:\Program Files\Common Files\Microsoft Shared\web server
extensions\50\isapi.  _cti_rpc appears to be a call within the .dll.

Only port 80 is open through ISA to the web server.

I have a MSDN Universal subscription...  removing my cross-posted NGs from
the list and then telling me to post in a group I already did, which has yet
to be addressed, does not satisfy our agreement of you [MSFT] responding
 to
my query within 48 hours.  It's been 3 days.  Where's the love?

Where is the agreement that says MS must respond to a public newsgroup
within 48 hours?

They are peer-to-peer newsgroups, not MS support or MSDN support.

It's peer-peer unless you have a MSDN Universal Subscription.  You should
read up.

About discussion groups

That service is available only at MSDN Managed Newsgroups

You are posting / crossposting to MS Public newsgroups
- which are not a part of that MSDN service
- which are peer-to-peer newsgroups w/ no guarantee of response

I am doing as I have always done.  However, do this...

click the second link you posted... and search for the subject of this
thread: FPSE - ISA - IIS - VS6
Why would a public post show up in a managed newsgroup?

In fact, using the web-based alternative, which I don't use:
Web Development/Internet Server Programming/inetserver.iis is actually
m.p.inetserver.iis.  You put your mouse over it and it shows on the status
bar.

Now perhaps the frontpage and ISA ones I included are not supported as
managed but clearly iis is so I'll remove the other two.  I have no idea
which group you're seeing it on but I've set the follow ups to just the iis
group.
