I have created a form and want the results email to me.  After submitting th
e
form, a prompt appears which asks for a usercode/password.  Why?  Help.

Sounds like you also set the form to save results to a file
(in a folder which requires write permissions_
By default the _private folder allows form writes

How do I take out the save option as well?

Under Form Properties, remove the filename from the save field...

It is blank.  The only item compelted is the email address.

Hi Doug,

The FrontPage form requires the FrontPage Server Extensions and publishing
to http location.

If your host server offers the extensions, I would try reinstalling them and
then republish. If you continue to have problems, a URL for the form page
might help.
