We have already applied the hotfix for IIS 6.0 that is explained in article
218180, and the server has been rebooted, but our server is still passing th
e
internal IP address in the header info. What could we be missing?

Forgot to mention, SP1 is already installed as well. This hasn't fixed the
problem either.

Have you followed ALL instructions in:
http://support.microsoft.com?id=834141

Especially the UseHostName and SetHostName properties?

Basically, you must apply the hotfix and also set this property to tell IIS6
to "use this hostname" instead of using the internal IP. If you don't set
the property, you get the original behavior even with the hotfix.

Because... IIS has to report *something* ... and depending on configuration,
it could be internal IP. The hotfix allows you to tell IIS what value to
report when necessary.
