You have 5 form tags on your page.
You need to have a single set of form tags with all of the form fields and t
he submit button between
the open and closing form tags. You also must publish the site to a server t
hat has the FP
extensions and is configured to process email via FP's http mode if you want
to use the FP Form
Handler.
