1) it is for VS.NET 2003 and VS.NET 2005 after SP2.
2) you are right: you need to use HTTP. Otherwise IIS is not involved and
the ISAPI filters from MCMS will not be loaded.
So only HTTP is support for MCMS.
The "mini" IIS in VS.NET is not a full IIS and is not sufficient for MCMS.
You cannot use with file system mode you have to use HTTP mode.

First of all I want to say thank you very much for all of your help.
I had success with the patch installation, but came to one realization.
Before I explain I have to ask a couple of questions...
1. Did the patch relating to

Missing" only deal with Visual Studio 2003 and MCMS 2002 SP2 ? (I am
probing Microsoft for this answer too...)
2. After copying the templates to the appropriate Visual Studio 2005
folders within My Documents I ran "devenv /setup". I then opened Visual
Studio 2005, Selected File / New Project / Website and Selected a MCMS
Template. What I did from the beginning was "always" choose to create a
new website using the "File System" and not the "HTTP".
That is why I could not see the "MCMS_Template Explorer" menu option.
When I started choosing "HTTP" to create a new site I was able to
select "MCMS_Template Explorer" (or that is when it appeared). My first
instinct was to ask for help (search on the web / contact you etc...) I
did find a problem relating to "MCMS Template Explorer Missing" both in
your forum and Microsoft...
My guess is that I always had the ability to do this. Maybe the patch
was needed but that is why I ask the two questions above. I am
reverifying to see if the patch was needed from the beginning (in my
case).
Generally, I do create websites, in Visual Studio 2005, by selecting
the "File System" and working that way. I really never use "HTTP" since
Visual Studio 2005 has it's own little web server built in.
This may have been a learning curve issue... Most likely.
