I'll check that versions match.

Unity 4.0(4)

1) check to make sure the AC client version is the same as the AC server
version. I.e. HELP>ABOUT

2) are you using unity or another voicemail solution?

you could start with AC client traces on the PC,

you could get the TCD traces from the CM server

or TCD talks to CTI so take the SDL\CTI traces and search for 0x8 to
find errors there

or you could look at the CTILineRedirectReq and CTILineRedirectRes
signals in SDL\CCM traces

whatversion of CM? and how is AC configured to know the VM Pilot number
for that user. In older CM versions you had to configure an ini file,
in newer versions we do a database dip to get the profile and pilot
number.

In Attendant Console I get a call, lookup a user in Corp Directory,
right click on that user then choose voice-mail. It fails to transfer
thecaller to voicemail.

Ideas on where to start troubleshooting?

I can transfer callers to extensions with no issues, just right click
voicemail fails.
