Found the problem, we had to follow MS KB 292509 to get it working with
exchange 2003, in addition to the steps in the help file.

Anyone used Unity Migrate Subscriber Data tool with Exchange 2003? Having
some difficulties getting the permissions correct, i'm curious if its even
supported on exchange 2003 or not, the docs don't really say that I can
tell. We've followed the instructions in the help file for exchange 2000 but
its still tossing up the "send as" "recieve as" error.
