Has anybody had problems with LTO or VXA tape drives?

I know I've had problems  between DLT tapes and DLT drives with the
little loop.  if it not just in the right spot, the drive gets jammed
with the tape in it and you have to send it in for service to have them
separated.  sometimes  you end up doing that at random times anyway.
