Make sure you have published the form using http mode to the server and that
the host has enabled email transport in addition to the
FP SE
PS
Most hosts will not allow forms to be sent out of your domain
(like someone@bayoftheholyspirit.com)
- yours is going to S-Email-Address="swf@memtime.net"
