This happened on two servers.

SQL Server is remote to BizTalk, right?  If so, make sure the MDAC versions
match on both machines.  You can update MDAC on your BizTalk machine to at
least 2.7.

The problem was related to MSDTC.  Because the new server was a ghost
of another server the SID was an issue.  Uninstalling MSDTC and
reinstalling it solved the problem.
