This looks like the text from _derived/nortbots.htm, which page is
(normally) served when a form configured to use the FrontPage extensions is
used from either a disc-based web, or the form page (or entire web) has been
FTPd to the server instead of HTTP published.
If the entire web was FTPd, then the FP extensions on the server will be
corrupted and must be repaired or re-installed.
If only the page was FTPd, then open the live web (File->Open web/site, then
type in http://www.domain.com/ for the destination) in FrontPage and
Tools->Recalculate Hyperlinks may repair the form, if it doesn't the page
will have to be republished using FP Publish command and using
http://domain.com as the destination address (NOT FTP).
