I DON'T KNOW HOW TO FILL IN THE  INFORMATION TO ACCESS THE FORM RESULTS BY
EMAIL (jUST EMAIL)  i DON'T WANT IT SAVED IN THE PRIVATE FOLDER ON THE WEB
SERVER.  I DON'T KNOW WHETHER TO USE THE HTML FORMAT OR THE FORMATTED TEXT
TAG.
