If you do not plan on using BAS (Business Activity Services) the you do not
have to install Sharepoint
(either WSS or SPS). To the best of my knowledge, HIPAA/HL& accelerators do
not use Sharepoint,
so you could run setup and do a custom setup without BAS.

SPS is built on top of WSS. BAS can work with WSS or SPS.
When used with SPS, it will be able to "talk" only to WSS so it will not be
possible to have a BAS site in
an area.
Again, if you do not use BAS, then you do not need Sharepoint (WSS or SPS).
